Instructions: Daystar only accepts applications from non-profit organizations. Please download and print this application, and complete it in its entirety. We are unable to consider partially completed applications. Please mail or fax your completed application to our screening committee, along with a copy of your 501c3 non-profit status, a statement of faith, and a mission statement to:

Upon receipt of your completed application, and if your program appears to meet Daystar’s programming needs, you will be contacted via email to submit a sample program DVD to our program evaluation committee. Sample programs are evaluated for content, ministry value, and technical quality. Upon program approval, you will be contacted to discuss time avails and rates. Please do not submit a sample program with this form.

Note: If you are an agency or production house submitting a proposal for a client or ministry, please complete the following section with CLIENT or MINISTRY information, not your company information.
